Senior Case Manager
Working in collaboration with the Supervisor, this candidate will be the hospital liaison, insuring strict adherence to protocol and assist with level of care changes, working with PCM to prevent hospitalization as well as outreach for non-engaged consumers. Will be responsible for intake meet and greets and assist with crisis intervention to facilitate smooth transition & effective interventions, and will also assist with staff coverage during periods of time off/PTO, etc. Maintains case load as indicated by consumer census and team need. Participates in rotating on-call schedule and creates the monthly on-call schedule for the team. Must have knowledge of co-occurring modalities including the ability to engage, screening, motivational interviewing and treatment matching protocols. Must be able to multi-task in a fast-paced environment.
Master’s in Social Work, Psychology or Counseling with appropriate credentials/licensure. Supervisory experience a plus. Familiarity and experience working with CSM Team consumers preferred.
