Senior Case Manager
Working in collaboration with the Supervisor, this candidate will be responsible for discharge protocols, process summaries, outreach for non-engaged consumers and medication extension requests. Attendance with self-determination, review of appropriate authorization and assist with PCM coverage. Maintains case load as indicated by consumer census and team need. Participates in rotating on-call schedule and creates the monthly on-call schedule for the team. Must have knowledge of co-occurring modalities including the ability to engage, screening, motivational interviewing and treatment matching protocols. Must be able to multi-task in a fast-paced environment.
Master’s in Social Work, Psychology or Counseling with appropriate credentials/licensure. Supervisory experience a plus. Familiarity and experience working with CSM Team consumers preferred.
