Provide Autism case management services in home and/or community to include intake and annual assessments, develop person-centered plans and service reviews, maintain clinical records to include electronic health record. Adhere to timelines and ensure authorizations are pre-approved. Uphold productivity standards, refer for ancillary services (respite, applied behavior analysis, occupational therapy, speech therapy, physical therapy, etc)
Must have behavioral experience and experience working with individuals with special needs. Position has flexibility, however, does not have traditional work hours, may require some evenings.
Requirements:
- Bachelor's degree in human services field
- At least one year experience as Case Manager or Support Coordinator
- One year experience working with individuals with developmental disabilities
