We are seeking a dedicated ACT Case Manager to join our Assertive Community Treatment (ACT) team. In this role, you will support individuals living with severe and persistent mental illness by helping them build independence, stabilize in the community, and improve their overall quality of life. ACT services are community-based and person-centered, meeting individuals where they are and supporting them at times and locations that work best for them.
Responsibilities
- Provide community-based case management services to individuals with severe and persistent mental illness
- Support individuals in developing independence, stability, and community integration
- Coordinate person-centered services that connect individuals to medical, social, educational, and community resources
- Assist individuals in identifying and developing natural supports within their community
- Foster an environment of acceptance, empathy, and hope, while promoting dual recovery and improved quality of life
- Provide services primarily in the community, based on the individual’s preferences for time and location
- Collaborate with the ACT interdisciplinary team to ensure coordinated care and support
Requirements:
Education
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related human services field required
Licensure
- Professional licensure preferred, but not required
- If licensed, license must be valid and in good standing
Experience
Minimum of 1 year of experience in community mental health or a related human services
