Senior Supports Coordinator

Provide Senior Case Management assistance to the Case Management team to include: assistance with training of new staff, review and sign clinical documents, assist with authorizations when needed, provide on-going support, supervision and training to team members, problem solving and crisis intervention and assist Support Coordinator Supervisor. This position will carry a caseload of approximately 30 support coordination consumers along with the responsibilities listed above.

Bachelor’s Degree in human services field. Must have experience working as a Supports Coordinator/Case Manager.
