Wraparound Facilitator
Services provided to children 4-17 years of age. Assures that all service professionals and support persons work together and assist in coordinating activities. Child/Family Team will create highly individualized wraparound plan with the child/youth and family that consists of mental health specialty treatment, services and supports. Assist youth and their families to achieve a life reflecting their hopes and dreams. Intensive, individualized care planning to achieve positive outcomes by providing a structured, creative and individualized plan. Advocating for youth and families by accompanying them to appointments relevant to achieving objectives. Provide information, education and guidance related to effective communication, problem-solving, anger management and child development. Education and support to enhance capacity to improve quality of life by assisting with locating and utilizing community resources, services and activities. Assistance in improving skills related to stress management, family budgeting, home maintenance, health and nutrition.
Required: Bachelor’s degree in human services field required.
