Provide community based case management services to adults with mental illness or co-occurring disorders of SPMI/SA. Familiarity with specialized residential providers, AFC and institutional settings is a plus. Provide annual assessments, person-centered planning, updates the plan annually or with changes in levels of need. Familiarity with co-occurring modalities including ability to engage, screening, motivational interviewing and treatment matching protocols preferred. Computer proficiency required in MS Word/Excel. Ideal candidate must be able to multi-task in a fast-paced environment.

Bachelor’s degree in human services field required.
