Case Manager/Therapist
Responsible for case management, individual therapy and facilitating groups. Therapy generally addresses crisis resolution, therapeutic stabilization and improvement in adaptation. Required to know and effectively use a range of therapeutic methods currently endorsed by standards of best practice and provide crisis intervention. Will provide community-based case management services to individuals with mental illness or co-occurring disorders. Provide initial and annual psychosocial assessment. Person-centered planning, updates the plan annually or with change in level of need. Must be familiar with co-occurring modalities including ability to engage, screening, motivational interviewing and treatment matching protocols.
Master’s in Social Work with licensure (LLMSW/LMSW) required.
